Creating A Form/Survey
Step 1: Access the Forms and Surveys Feature
Log into your DOJODIGI account and navigate to the “sites” tab and click on forms or survey page.


Step 2: Create a New Form or Survey
Click on the “Add Form” or "Add Survey" button to start a new form or survey. Choose whether you want to create a one from scratch or use one of our many professionally designed templates for ease and convenience.
Step 3: Customize Your Form or Survey
Similar to our website and funnels editor, our forms and surveys editor/builder is a convenient drag and drop editor.
Editing elements
Click on the + button to add the form/survey fields.

To edit any element in the form or survey, simply click on it. The settings for that element will appear in a popup column on the right, allowing you to make changes without modifying the main survey column directly.

To add in custom survey/form questions, click on the custom fields tab.


Custom Fields allow you to gather and store specific data points about your contacts that go beyond the default fields in DOJODIGI. In the forms and surveys context, they are essentially your unique survey or form questions. After adding a custom field, it will appear under the "Custom Fields" section in the form and survey builder. Read more on custom fields here.
To add a new slide/page to the survey, click on the + icon below
NOTE: Only surveys allow for multi-slides/pages

Form/Survey General Settings
Click on the settings button on the top right of the editor

You can edit the general settings of the form or survey here, including:
Styles: Add footers, custom CSS, fonts, colors, etc.
Themes: Apply templates to change the overall look and feel.

Advanced Options: Configure settings like submission messages, disqualify messages, pixel ID insertion, and survey-specific settings (e.g., one question at a time, sticky contact, disable auto-navigation, etc).

To edit the footer, click on "styles" and scroll down to the footer section

Form/Survey Notifications
Click on the bell icon on the top left to toggle your survey/form notification settings

Custom Logic & Conditioning (Forms Only)
Click on the conditions icon on the top left

Custom logic in DOJODIGI's Forms Builder allows you to create dynamic, responsive forms that adapt based on user inputs. This enhances the user experience and ensures that the right actions are taken based on specific conditions. Here are some key custom logic features and their purposes:

Implementing DOJODIGI Form/Survey on a DOJODIGI Website
Inserting directly into a section
In the sites tab, navigate to the websites page and select the website and website page of choice to implement the form/survey.
Drag and drop the form/survey to the location of choice

Do note that the form/survey will show up exactly how it is. Keep this in mind when designing your form/survey to match the rest of the site
Other Ways to Insert Your Form (Layout type)
In the form editor, click on "integrate"

Select your embed layout type, activation options, deactivation options and trigger type. Click on "copy embed code"
In the sites tab, navigate to the websites page and select the website and website page of choice to implement the form
Drag and drop a code to anywhere on the site
Open the code editor and paste the copied code

Implementing DOJODIGI Form/Survey on an External Website
Note the exact integration steps may vary depending on your CMS/Website builder.
Click on "integrate" in the editor

Login to your CMS/Website builder
Navigate into the website editor
Scroll through your site settings and find a section that says "edit footer code" or "edit body code"

Or for forms/surveys that you want at a specific spot on your website, insert a custom code there and paste the copied code

Paste the code into the editor and save

Your DOJODIGI form/survey is now added to your website

In the forms or surveys page in the sites tab, hover over the drop down menu to access the analyse and submissions page. View data on your forms/surveys and their submission details here.
